  • *Certified Copy* of players Birth Certificate

    • Returning Lakers do not need to submit​

  • Copy of current Sports Physical

  • Signed Policy Manual

    • Will distribute when available​

  • *Notarized* Player Indemnification Form #2

    • Will distribute when available​

  • $75 Registration Fee
